The Adair County SB40 Developmental Disability Board is seeking a Service Coordinator to provide targeted case management services, within a person-centered support and planning framework, for persons with developmental disabilities. For a caseload of approximately 35-40 people, under the TCM program, service coordinators are responsible for ensuring that individuals receiving supports have access to and receive services that meet their needs. This position will be also responsible for monitoring the effectiveness of the services and supports being given. The position requires one of the following qualifications: a registered nurse license or a Bachelor’s degree from an accredited college or university.
